Summary of Reevaluation Changes by Document

Part A: General Project Identification and Description

Since the approval of the evaluation in August 2022, the Project Location Map has been updated to reflect the additional work, the Design Plan attachment has been updated with the current design. The Project Description has been revised to include additional work. This additional work includes reconstructing approximately 700 feet of pavement along SR 4035 (South 4th Street) between the intersection with SR 0061 south of the intersection with Hawk Ridge Drive. An additional 450-ft of reconstruction will occur north of the intersection with Hawk Ridge Drive to include a left turn lane to accommodate the installation of a traffic signal at the South 4th Street & Hawk Ridge Drive intersection. Supporting work will include sidewalk adjustments, drainage installation, driveway adjustments, and guide rail upgrades. Additionally, all weather pavement markings (AWPM) will be installed throughout the entire project limits (MPMS 97258, which has been added to the CE).

Part B: Section A-3

Since the approval of the evaluation in August 2022, the PNDI was updated with the revised Project Study Area, which includes the additional work along South 4th Street. The PNDI was re-run on December 30, 2025. The updated receipt indicated potential impacts to threatened and endangered species under the purview of the PA Fish and Boat Commission (PFBC) and the US Fish and Wildlife Service (USFWS). The PFBC species was not disclosed on the PNDI, but coordination with the PFBC, concluded on 12/30/2025, indicated that there are no adverse impacts to the species of special concern from this project. The updated clearance letter is attached. The PNDI additionally requested more information regarding two species under the purview of the USFWS. Due to the proximity of the project to a known bald eagle nest, it was requested that a "Bald Eagle Project Screening Form" be completed, and any specified measures be implemented. The Screening Form was completed. The project meets the category of Maintenance and Restoration Activities for linear utilities, road, bridge or culvert maintenance. Avoidance measures were recommended to avoid disturbing nesting bald eagles. A copy of the completed Screening Form can be found in the Project Technical Files. The PNDI also requested that the project be run through the USFWS IPaC system. Based on the results of the IPaC, the project is in the range of the Federally Endangered Indiana Bat and the Proposed Federally Endangered Tricolored Bat. The project was run through the determination key for FHWA, FRA, FTA Programmatic Consultation for Transportation Projects affecting IBAT, NLEB, or TCB and a determination of May Affect-Not Likely to Adversely Affect, as long as the recommended AMMs are implemented. The concurrence letter is attached. The bog turtle is no longer listed on the PNDI receipt, as the species range has recently been updated. As such, no further coordination regarding the bog turtle is necessary and the bog turtle AMMs are no longer required

Part B: Section A-7

Since the approval of the evaluation in August 2022, the total number of parcels that require right-of-way acquisition, either partial or total, has increased from 32 parcels to 36 parcels. Additionally impacted parcels include 75, 126, 139, and 147, due to driveway adjustments, drainage installation, and the additional work along South 4th Street. The number of parcels that will require ROW acquisition increased from nine parcels to twelve parcels. ROW acquisition will now be required from Parcels 86, 139, and 147, as a result of drainage installation and the additional work along South 4th Street.

Confirmation of Reevaluation Classification and Level

Upon reevaluation, as supported by the attached documentation, this project qualifies as a CE Level 1b.
