Part A: General Project Identification and Description
Changes From Approved May 2023 CE1b: On March 25, 2025, the project's Limits of Disturbance (LOD) were expanded slightly to accommodate the installation of Muscle Walls to prevent flooding during project construction. Located on the southeastern bank of the Youghiogheny River, these Muscle Walls are temporary and will be removed at the conclusion of the project. The resulting changes have been made to this section: Project Limits- End limits changed from offset 0060/0255 to 0060/0422 and station 481+60.00 to 483+27.00 Attachments- Updated Project Location Map and Construction Plans

Changes From Approved May 2023 CE1b: -Permanent stream impacts updated from 69 to 104 linear feet, and stream impact details (permanent and temporary) updated. -JPA noted as approved 11/07/2025 -Floodplain Mitigation changed to Yes, with description of Muscle Walls on southeastern bank of Youghiogheny River (Stream 1) included. -ESCP noted as approved.

Changes From Approved May 2023 CE1b: Hazardous Waste updated to note Phase II/III sampling occurred in November 2025, and the Phase II/III ESA has been approved (March 2026) and is included in the project technical file.

Part B: Section A-3
Changes From Approved May 2023 CE1b: Due to the two year expiration, the PNDI has been rerun and the updated receipt is attached.

Part B: Section A-4
Changes From Approved May 2023 CE1b: On March 25, 2025, the project's Limits of Disturbance (LOD) were expanded slightly to accommodate the installation of Muscle Walls to prevent flooding during project construction. Located on the southeastern bank of the Youghiogheny River, these Muscle Walls are temporary and will be removed at the conclusion of the project. The walls will not impact any archaeological resources. While the Muscle Walls lie within the West Newton Downtown Historic District, they are temporary and will not physically impact any aboveground resources that contribute to the historic district. For these reasons, they will not diminish any of the characteristics that qualify the district for listing in the National Register of Historic Places. The project finding remains No Adverse Effect. Please open the gate for submission of the update APE shapefiles.

Part B: Section A-5
Changes From Approved May 2023 CE1b: Due to design changes, West Newton Historic District Section 4(f) impacts have changed from 0.421 to 0.470 acre of temporary impacts (permanent impacts have not changed). Youghiogheny River Water Trail Section 4(f) impacts have changed from 1.189 to 1.187 acre of temporary impacts and 0.013 to 0.010 acre of permanent impacts. The West Newton Historic District No Use form and the Youghiogheny River Water Trail de minimis form have been updated and signed forms are attached.

Changes From Approved May 2023 CE1b: The project is exempt from Detailed Project Level Community Impacts Analysis. A Section 2002(b) Community Impacts Documentation Form has been completed, and no known disproportionately high and adverse effects to project area communities would result from the project. The form is included in the project technical file.

Part B: Section C
Changes From Approved May 2023 CE1b: A second public officials meeting was held on February 10, 2026, and a second plans display was held on February 19, 2026. The meetings presented final construction plans, traffic control, proposed bridge renderings, and the projects anticipated construction schedule. No opposition to the bridge rehabilitation project was expressed.

Changes From Approved May 2023 CE1b: The Joint Permit Application (Permit Number E6505125-002; KEES Application Number 16804) was approved on 11/07/2025.
