Part A: General Project Identification and Description
Project description updated to: “The project is located in Bullskin and Upper Tyrone Townships, Fayette County, and consists of a safety project and roadway reconstruction. It involves the construction of a new grade-separated access midway between the intersections, which includes acceleration and deceleration lanes along S.R. 0119 northbound; a new connector road to S.R. 1027 (Mount Pleasant Road), including a roundabout along S.R. 1027; an extension of T.R. 791 (Kendi Road); acceleration and deceleration lanes along S.R. 0119 southbound to improve safety and maintain access for the right-in/right-out movements at both McClure Road and Kingview Road; a deceleration lane along S.R. 0119 northbound to improve safety and maintain access for the right-in movement at Crossroads Road; and acceleration and deceleration lanes along S.R. 0119 northbound to improve safety and maintain access for the right-in/right-out movements at McClure Road.”

Part A: Engineering Information
Maintenance of traffic description added under "Additional Information": The initial phase of construction includes the extension of Kendi Road (requiring no traffic control), construction of the overhead bridge, Connector Road, and roundabout (roundabout requires a detour of Mt. Pleasant Road traffic), reconstruction of Kingview Road, McClure Road, and Crossroads Road (requiring phasing and 24-hour flagging operations). Following completion of the off-line roadway network construction, Phase 2 begins with construction of temporary Route 119 median pavement. Singel lane traffic is maintained on Route 119 in each direction. Phase 3 continues with reconstruction of Northbound Route 119 lane with traffic maintained on either the SB Route 119 existing lanes or, NB Route 119 on the temporary median pavement. Phase 4 continues with reconstruction of Southbound Route 119 lane with traffic maintained on either the new NB Route 119 lanes or, SB Route 119 on the temporary median pavement. Phase 5 continues with reconstruction of the Route 119 median with both NB and SB lanes of traffic maintained on their respective new construction.

Part A: Roadway(s)
Each roadway verified for correct information and T-791 updated for existing lane width: Existing Lane Width to 11 ft, Proposed Lane Width to 12 ft.

Part B: Section A-1
Streams and wetland sections updated to reflect current impact and mitigation numbers as presented in the latest iteration of the permit submission to PADEP: Streams: • Both 1,516 LF and 1,589 LF were from early iterations of both impact calculations and the draft permit numbers. The impact has been updated to 2,280 LF based on final design, as presented in the latest iteration of the permit submission (March 2025). Coordination with PADEP regarding the permit and mitigation is ongoing. • 351 LF of open channel loss will be compensated by 374 LF of mitigation (STR 05). • Additionally, 623 LF of stream (STR01, STR04, and STR07) will be restored to pre-impact conditions. Wetlands: • Updated description reflects a total of 0.64 acres of wetland (consisting of 4 wetlands, including wetland 5) will be permanently impacted. • Wetlands 1, 2, 3, and 5 are permanently impacted, but only three will result in permanent habitat loss. • Mitigation totals 0.75 acres of onsite, replacement wetland. Coordination with PADEP regarding the permit and mitigation is ongoing. E&S Control Plan and NPDES Stormwater Construction Permit have been selected. EM Request 1/8/25 Additional details added regarding impacts to streams, including adding impacts to ephemeral streams and adding design reasoning for impacts. Note added that coordination with PADEP regarding permit submission is ongoing.

Part B: Section A-2
The additional ROW areas (wetland mitigation site and design shifts) were confirmed to be included in the final version of the FAR. The FCIR was updated in July, 2024 to reflect the wetland mitigation site and design shifts and the NRCS response letter is included as an attachment. Waste section updated to yes for “Is remediation/mitigation incorporated?”

Part B: Section A-3
PNDI has been rerun to include the new wetland mitigation area and a comment reflecting that was included. New PNDI attached.

Part B: Section A-4
Part B Section A-4 encompasses the updated correspondence with the State Historic Preservation Office (SHPO).

Part B: Section A-6
Air Quality updated to confirm that the project is on the approved 2025-2028 TIP from SPC. Noise: Details on the October 2024 and subsequent March 2025 Final Design Noise Study and approvals are included in the Noise Remarks. Approval dates from PennDOT CO and FHWA added, and the new transmittal letter included as an attachment.

Part B: Section A-7
Review completed and property impacts were confirmed. This section has been updated to reflect changes in RROW information. Confirmed that no commercial relocation is required. Project has 56 total ROW claims for 55 parcels. One parcel had two claims.

Part B: Section C
Added that public meeting was held 11/25/24 and provided brief reasoning. Checked off and added reference to ProjectPATH website and summarized tribal coordination for Section 106 Public Involvment Activities.

Part C
Updated preparer information/date.
